MyChart offers parents personalized and secure on-line access to portions of their child's medical records. It enables you to securely use the Internet to help manage and receive information about your child's health.With MyChart, you can:

No, MyChart is a free service for all patients.

Parents who wish to participate will be issued a MyChart activation code during their doctor's office visit. This code will enable you to login and create your own user ID and password. If you have not been issued an activation code, you may ask your doctor's office for one during your next visit.

What do I need to use MyChart?

You need access to a computer or mobile device connected to the Internet and an up-to-date browser (such as Edge, Chrome, Firefox, or Safari). You can also use the MyChart mobile app on a mobile device.

How is MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account must be accessed using that password. On mobile, the account may also be accessed using face ID or fingerprint login. Unlike conventional email, all MyChart messaging is done while you are securely logged in to our website or mobile app.

You can ensure that your account stays secure even if someone else has your username or password by turning on two-step verification. When this feature is turned on, you must enter a code that is sent to you by email or text message to log in to MyChart, in addition to using your username and password. What is your Privacy Policy?

MyChart is owned and operated by Rady Children's Hospital-San Diego and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold, leased or otherwise distributed. What is MyChart for iPhone or Android?

MyChart for iPhone or Android includes many of the core features available in the Web-based version. We plan to make additional features available in future releases. The modern user interface was designed specifically for use on the Apple iPhone, iPod touch, iPad, and Android smartphones. With MyChart for iPhone or Android, you can:

  • View your child’s test result values and standard ranges for each result.
  • Create new medical advice request messages using the Get Medical Advice feature. You can also view, delete or reply to messages in your Inbox.
  • View upcoming appointments, confirm or cancel them, and view clinic information, such as the phone number and driving directions. You can also view the After Visit Summary for past appointments, excluding any discharge attachments.
  • View information in your child’s health summary, including medications, allergies, immunizations and current health issues.

Users with proxy access can view all of the features above for patients to whom they have access. They can also customize the photo and background color that appears for each family member.

My activation code does not work. What should I do?

For your security, your access code expires after 7 days and is no longer valid after the first time you use it. If you still have problems, email us at MyChart@rchsd.org or you can call the Rady Children's Service Desk at 877-902-4278.

Your Child's Medical Record

How do I request copies of my medical records using MyChart?

After logging into MyChart click on the Request Medical Records button in the center of the page. An electronic form will come up which you may then fill out with the specifics for your request and then click the Submit button at the bottom of the form to send your request to our Health Information Department. They will respond to your request within 14 business days.

When can I see my test results in MyChart?

With MyChart, you can view most test results as soon as they become available. Know that you will likely see results before your healthcare provider has had a chance to review them. After your provider reviews your results, you might see additional comments and interpretation in MyChart.

Your provider is able to determine which types of test results are able to be accessed through MyChart. Further, tests of a very sensitive nature are not released to MyChart.

If I send a message to my doctor or nurse, when can I expect a reply?

You will generally receive an answer within 3 business days. Please note that MyChart should not be used for urgent situations. Please contact your doctor's office if the situation requires immediate attention or dial 911 if it is an emergency.

Where can I update my personal information?

Log into MyChart. From the main menu click Personal Information to edit your information.

How do I change my legal name in MyChart?

Preferred name changes can be made at any time in MyChart by accessing the main menu and selecting Personal Information. Any changes to the legal name can be made by visiting your doctor’s office and bringing identification with updated information.

If some of my child's health information on MyChart is not correct, what should I do?

MyChart information comes directly from your child's electronic medical record at the doctor's office. Ask your doctor to correct any inaccurate information at your child's next office visit. Your child's health information is reviewed and updated in their electronic medical record each visit.

MyChart for My Family

Can I view a family member's health record in MyChart?

Yes, you can. This is called proxy access and allows a parent (or guardian) to log in to their personal MyChart account and then connect to information regarding their family member. You must be at least 18 years old to have proxy access to another person’s medical record.

You might also want to grant a family member or friend access to your medical records when you need assistance managing your appointments and other medical needs. To give someone access to your medical record, grant access via a proxy invite through the Friends and Family Access page in MyChart.

After I have enrolled

I forgot my password. What should I do?

You may click the "Forgot Password?" link on the sign-in page to reset your password online. You may also contact the Rady Children's Service Desk at 877-902-4278 to request a new, secure password.

Where can I update my child's personal information (e.g., home address, e-mail or change my password)?

Log into MyChart and from the left menu, go to the Preferences section and select the appropriate option.

I was logged out of MyChart, what happened?

We aim to protect your privacy and security of your child's information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

I have multiple MyChart accounts. How do I link them together?

If you've been seen at another healthcare organization, you might be able to view information from that medical record right in MyChart. You might have heard of this feature referred to as Happy Together. This includes information from other organizations, such as:

  • Allergies
  • Care team
  • Health issues
  • Medications
  • Messages
  • Test results
  • Visits

To view this information, you must link your account. Check out our video tutorial on how to do this.

How do I delete my account?

We strongly encourage all patients to keep an active MyChart account, even if no longer receiving care at Rady Children’s Hospital. An active MyChart allows you to quickly access current and past communications with your care team, send messages to providers, view lab results and immunization records, save and print after visit summaries, view and request medication refills and request medical records. To deactivate your account, login, and access Security Settings from the main menu. At the bottom of the page, you will see the section titled Deactivate Your Account with a link to the account deactivation page.
